From Symptom Management to Ocular Surface Regeneration
The Scientific Rationale for MSC-Derived Exosomes in Dry Eye Disease
The future of regenerative medicine lies in understanding cellular communication and its role in tissue regeneration. Opublu Biologics advances this field through exosome technologies and mesenchymal stem cell (MSC) platforms, bridging scientific innovation from OEM manufacturing to clinical translation through strategic partnerships and proprietary intellectual property.
The Scientific Rationale for MSC-Derived Exosomes in Dry Eye Disease
From Symptom Management to Ocular Surface Regeneration
Opublu Biologics Scientific & Clinical Translation Series
Dry Eye Disease (DED) is a multifactorial disorder of the ocular surface involving tear-film instability, inflammation and progressive disruption of ocular surface homeostasis.
While conventional approaches such as artificial tears, lubricants and anti-inflammatory therapies remain important components of clinical management, increasing scientific attention is being directed toward regenerative approaches capable of addressing both inflammatory signalling and tissue repair.
Understanding the Immunoinflammatory Cycle
Persistent ocular surface dryness may result in epithelial injury and activation of inflammatory pathways involving dendritic cells, macrophages and T lymphocytes.
Pro-inflammatory mediators including TNF-α, IL-1β, IL-6, IFN-γ and IL-17 may contribute to a continuing cycle of inflammation, epithelial injury and impaired ocular-surface function.
This provides an important scientific rationale for investigating therapies capable of simultaneously modulating inflammation and supporting tissue recovery.
Scientific Challenges Remain
Despite encouraging preclinical research, MSC-derived exosome therapeutics remain an emerging field.
Important translational challenges include standardisation of exosome isolation and characterisation, batch consistency, source-related heterogeneity, scalable manufacturing, stability, storage conditions and establishment of appropriate potency and quality-control parameters.
Most importantly, the safety and clinical performance of individual exosome-based ophthalmic formulations must be established through appropriately designed clinical studies.
These considerations form an essential part of Opublu Biologics’ translational development philosophy.
MSC-Derived Exosomes
Mesenchymal stem cell-derived exosomes (MSC-Exos) are extracellular vesicles containing biologically active cargo that may include proteins, growth factors, cytokines, microRNAs and other signalling molecules.
The Emerging Role of MSC-Derived Exosomes
Preclinical Research
Preclinical research suggests that MSC-derived exosomes may influence several biological processes relevant to ocular-surface disease, including:

These properties have created growing scientific interest in MSC-derived exosomes as potential cell-free regenerative platforms for ocular applications.
Clinical Translation
From Scientific Evidence to Clinical Translation
Opublu Biologics is translating this emerging field into a structured ophthalmic development programme through LagoMed Oculogics™.
The programme is being developed to investigate the potential application of MSC-derived extracellular vesicle technology in Dry Eye Disease, with emphasis on scientifically controlled evaluation of safety, tolerability and exploratory clinical outcomes.
The objective is not simply to develop another symptomatic eye-care product.
Our longer-term scientific vision is to investigate whether regenerative signalling technologies can contribute to restoration of ocular-surface homeostasis.
Clinical Research
LagoMed Oculogics™ Clinical Research Programme
The proposed clinical investigation represents an important step in the translational development of LagoMed Oculogics™.
The programme is intended to evaluate areas including:
Safety & Tolerability
Assessment of ocular and treatment-related safety outcomes.
Clinical Response
Evaluation using established clinical measures of Dry Eye Disease.
Ocular Surface Recovery
Investigation of changes associated with tear-film stability and ocular-surface integrity.
Inflammatory & Translational Biomarkers
Exploratory investigation of biological signals that may help explain treatment response.
